This is an unusual medal of pilgrimage.  It features a coat of arms for Our Lady of the Treille.  The words translate to Saint Mary of the Treille Confraternity and Patroness of Lille.  This is a devotional item associated with the Cathedral of Note-Dame de la Treille in Lille, France.  The cathedral's name comes from a miraculous statue of the Virgin Mary that was protected by an iron trellis (treille in French).
