This is a well-loved vintage medal of Our Lady of Lourdes from France.  Our Lady appeared to St. Bernadette in the Grotto at Lourdes in 1858 where many miracles have occured.  On the back is the scene of St. Bernadette before the Blessed Mother.  A medal of healing graces.
